Infection Prevention & Control (IPAC) Lapse Disclosure

An infection prevention and control lapse disclosure report is created when an infection prevention and control lapse is discovered by a Huron Perth Public Health inspector. This often happens through the assessment of a complaint or referral.

The identification of a lapse is based on assessment and investigation of a business or facility at a specific point-in-time. These assessments and investigations begin when potential infection prevention and control lapses are brought to the attention of the local Medical Officer of Health. 

In accordance with the Ministry of Health's Infection Prevention and Control Disclosure Protocol, once an investigation is completed, a final report must be posted for one year from the time all corrective actions were completed.
